  • Waivers completed for Oasis Climbing or Urban Jungle

  • Direct conversations with participants and/or their parent/guardian

  • Session notes recorded by ClimbWell / Oasis Climbing staff

Only authorised ClimbWell / Oasis Climbing staff involved in your care or administration have access to your information.

7. Website, Forms & Waivers

When accessing ClimbWell services within Oasis Climbing or Urban Jungle:

  • You may be required to complete their facility waivers.

  • These are separate documents and managed by their respective CRM stsyems.

  • We do not receive your full waiver data; we only receive information relevant to your participation (e.g., whether a valid waiver exists).

Any digital forms completed through ClimbWell (e.g., intake forms or progress forms) are encrypted and stored securely.

8. Media & Photography

We do not share photos or videos of participants without consent.
Where consent is provided, images may be used for:

  • Progress tracking

  • Reports or clinical documentation

  • ClimbWell marketing materials (if explicitly agreed to)

You may withdraw consent at any time.
